If 60 years old and up, with Special Power of Attorney
Must have a stable source of income

BASIC DOCUMENTARY REQUIREMENTS
Completely filled up Application Form
2x2 picture (two copies)
Photocopy of 2 valid IDs (any ID with picture & signature, preferably government-issued)
Latest Proof of Billing (Electric or Water only, even if not under borrowers name)
Latest Proof of Residency (NBI or Police Clearance)
Sketch of Residence
Proof of Income
- PUV Operator: Updated OR/CR & franchise certificate
