2.) ? SUBMIT REQUIREMENTS
If the service address is feasible/serviceable/available confirmed, email back all CLEARED SOFT COPY of requirements.

•2.1.) Print & Fully Accomplished Application Form. Complete details with ORIGINAL signatures of Page 1(BACK) & Page 2(FRONT).
NOTE: Valid EMAIL ADDRESS & MOBILE NUMBER/s required.

•2.2.) Photocopy of BACK TO BACK 1 Government Valid ID w/ 3 ORIGINAL specimens signature of applicant.

•2.3.) Photocopy of applicant service address Latest Utility Proof of Billing (POB at least 60 days) – Under the name of applicant (No disconnection notice marked)

3.) ? PAYMENT
Application is still subject for Port Reservation/Validation/Approval, once the application was reserved & validated approved proceed for payment using the created applicant SUBSCRIBER ACCOUNT NUMBER. (email us the soft copy of PROOF OF PAYMENT {POP} to continue the process)

IMPORTANT NOTICE: SALES AGENT/S & INSTALLER/S ARE NOT ALLOWED TO ACCEPT PAYMENTS, PLAN INITIAL CASH-OUT SHOULD BE MADE THROUGH ACCREDITED PAYMENT CHANNELS.

4.) ? INSTALLATION
Once Job Order (JO) dispatched, installation process starts between 2 days to 10 working days (RESIDENTIAL) & 3 days to 20 working days (BUSINESS).
