6. Philippine arrival stamp re-entry (land and sea)
7. Marriage/Birth Certificate (land and sea)
8. Copy of latest allotment slip/certificate (sea)
9. Copy of Passport and Seamans book (sea)
10. Copy of seafarers Registration Certificate (sea) (SRC)
